Healthy Prestatyn Iach Boundary Update - 9th of September 2026 

Dear Patient,

We would like to share an update about the Healthy Prestatyn Iach Practice boundary.

We wrote to your household in June, to inform you that we were proposing a change to our Practice boundary.  We listened to feedback and made some changes to the boundary proposal, which are listed below.

The boundary change has been approved and will start on 1st January 2027.

You do not need to do anything now. Please continue to use Healthy Prestatyn Iach as normal.

 

Changes made after listening to patients

Thank you to everyone who shared their views with us at our engagement events, by phone, by email and through our online survey.

After listening to your feedback, we made some changes to the proposals:

  • The village of Gwaenysgor will remain within the new HPI Practice boundary.
  • Patients living in Trelawnyd, Trelogan and Llanasa who are registered with Healthy Prestatyn Iach will be able to register with Park House Surgery
  • Existing HPI patients living in Bodelwyddan to be able to register with the Practice that will take over the running of the Rhuddlan Surgery branch
  • Patients with complex health needs will receive an enhanced handover. This may include a face-to-face discussion between their current GP and their new GP to help ensure continuity of care

Important reminder: Patients who are receiving end of life care will remain registered with Healthy Prestatyn Iach regardless of their home address.

 

What happens next?

Nothing is changing until January 2027.

  • Keep using the practice as you usually do
  • You do not need to contact or move to another GP practice
  • Appointments, prescriptions and other services will continue as normal
  • Planning is taking place now to prepare for the changes. Patient transfers will begin after the boundary changes come into effect on 1st January 2027
  • The Health Board and local GP practices will work together to make these changes safely

 

Will this affect me?

A new boundary map is attached to this email and available on our website.  Patients living outside the new boundary will be affected and will be transferred to a new GP Practice in early 2027, unless they are exempt. 

Patients affected by these changes will be contacted by letter before anything changes.  These letters will be sent between December 2026 and April 2027.

This is because patients will move to their new GP practices in stages. Moving patients in smaller groups will help practices manage the changes safely, maintain continuity of care and complete any registration processes.

When a new GP practice has been allocated to you, we will send a letter to your home address, we will tell you:

  • Which GP practice you will move to
  • When the change will happen

Your medical records will be transferred securely between GP practices. You will not need to arrange this yourself.

 

How will I receive updates?

We will continue to share general updates through:

  • Email
  • Our website
  • Our Facebook page
  • Posters in GP practices
  • Posters in some local community locations

This is for general updates only, when you have been allocated to a practice we will write to your household.

Will I still have a GP?

Yes, everyone will continue to have a GP practice.

If the changes affect you, we will tell you which GP practice you will move to and when this will happen.

The boundary changes will take effect from 1st January 2027, but patients will move to their new practices in stages. This means you may receive information about your transfer at a different time from friends, family members or neighbours.

Until then, you will stay registered with Healthy Prestatyn Iach and continue to receive your care as normal.
