Ty Elan Hub
A one stop shop for diabetes annual review and follow up is now available for people with Type 1 or Type 2 diabetes who are registered with Healthy Prestatyn / Rhuddlan Iach to book into. This is a new North Denbighshire Cluster service. The appointments are currently offered at the Ty Elan practice in Rhyl.LL18 4HQ.
All of the essential screening and bloods / urine kidney checks will be completed on the day. Most of these results will be given to you during your appointment. You will also have the opportunity to discuss your results with a Chronic Conditions nurse who is skilled in diabetes. HPI has been given access to this service every Thursday. We are getting really positive feedback about this clinic.
Therefore we would encourage people who are due or overdue their diabetes annual review to take advantage of this service. Please contact us on the following number 03000 856980 on a Tuesday or a Thursday and we will be happy to book your appointment.
