Healthy Prestatyn Iach is reviewing its practice boundary as part of ongoing work to ensure GP services remain sustainable, resilient and able to meet the needs of local communities in the future.
The proposals aim to better align patient populations with local practice capacity, support care closer to home and strengthen primary care services across the area. No decisions have been made, and a formal engagement process is currently underway to gather feedback from patients, staff and stakeholders.
Patient safety, continuity of care and support for vulnerable patients will remain central to any future changes.
Below is the letter that has been sent to all affected patients.
